employment record and other matters related to my suitability for employment. I authorize, unless
indicated otherwise in this application, any person, school, my current employer and/or previous
employers in organizations contacted by JB Hudson to provide any relevant information regarding my
current and/or previous employment, and I release all persons, schools and employers of any and all
claims for providing such information. My signature below certifies that all information in this
application and the answers given by me during the interview process are accurate and complete to
the best of my knowledge and subject to verification by JB Hudson. I further understand that if, in
the judgment of the company, I have included any misrepresentation or omission of the facts or
circumstances that this false information will result in the refusal and/or termination of my
employment if discovered after date of hire. If hired, I understand that my first three months of
employment are a preliminary "Introductory Period". Further, I acknowledge that JB Hudson maintains
an "at will" employment relationship with all employees. I further understand that as JB Hudson
deems necessary, I may be required to work overtime or hours outside a normally defined work day or
work week. I also understand that I am required to abide all policies, procedures, rules and
